RE: NOTICE OF TERMINATION OF LEASE AND DEMAND TO VACATE AND PAY

Dear ________,

This letter serves as formal written notice, pursuant to the Contract of Lease executed between us and in accordance with the relevant provisions of the Civil Code of the Philippines (Republic Act No. 386), particularly Articles 1654, 1657, 1659, 1673, and 1687 thereof, of the termination of your lease over the property described below, for the reasons herein stated.

§ 1. The Leased Premises. This notice pertains to the residential/commercial premises situated at:

________ (the “Leased Premises”).

§ 2. The Lease Contract. Your lease over the Leased Premises commenced on ________ and is set to expire on ________, at a monthly rental of ________ Pesos (₱________), payable in the manner stipulated in the Contract of Lease.

§ 3. Ground for Termination. Pursuant to Article 1673 of the Civil Code and the stipulations of the Contract of Lease, I hereby exercise my right to judicially or extrajudicially terminate the lease prior to its expiry by reason of your breach of the conditions thereof, particularly the following:
