§ 1. Introduction. I, ________, son/daughter/wife of ________, aged ________ years, residing at ________ (hereinafter referred to as the "Consumer"), being a "consumer" within the meaning of Section 2(7) of the Consumer Protection Act, 2019, hereby address this letter to you in respect of the purchase of the product more particularly described herein below.

§ 2. Particulars of the Product. The Consumer purchased the following product from your Company: ________ (hereinafter referred to as the "Product").
