§ 1.  Retainer. We are the solicitors for ________ (the “Estate Trustee”), who is the estate trustee named under the Last Will and Testament of the Deceased dated ________ (the “Will”), and who is authorized to administer the estate of the Deceased (the “Estate”).

§ 2.  Assets Held. We are advised that your institution holds, at the branch located at ________ or at one or more of your branches, assets of the Deceased, including without limitation the account(s) and/or instrument(s) bearing the following particulars: ________ (collectively, the “Assets”).

§ 3.  Request. On behalf of the Estate Trustee, we request that you transfer the Assets into an estate account and thereafter release and pay the same to the Estate Trustee, for distribution to the beneficiary or beneficiaries of the Estate, without requiring a Certificate of Appointment of Estate Trustee With a Will (formerly known as letters probate or a grant of probate), having regard to the value of the Assets and your institution’s internal thresholds and policies.

§ 4.  Enclosures. In support of this request, we enclose the following:

1. a certified copy of the Will of the Deceased;
2. a copy of the Funeral Director’s Statement of Death and/or the provincial death certificate for the Deceased;
3. a copy of valid government-issued identification of the Estate Trustee; and
4. such further documentation as is itemized at ________.

§ 6.  Authority and Privacy. The Estate Trustee, as the duly appointed personal representative of the Deceased, hereby consents to your institution disclosing to the undersigned all information respecting the Assets. The undersigned is authorized to make this request and to receive such information on behalf of the Estate Trustee.
