Sehr geehrte Damen und Herren,

1. hiermit kündige ich, ________, den zwischen mir und Ihnen bestehenden Mitgliedschaftsvertrag vom ________ ordentlich und fristgerecht zum ________, hilfsweise zum nächstmöglichen Zeitpunkt.

2. Die Kündigung umfasst zugleich sämtliche im Rahmen des Vertrages vereinbarten Zusatz-, Neben- und Sonderleistungen, die zum selben Zeitpunkt enden sollen.

4. Ich bitte Sie, mir den Zugang dieser Kündigung sowie den genauen Zeitpunkt der Vertragsbeendigung gemäß § 309 Nr. 13 BGB in Textform schriftlich zu bestätigen.
