I am writing in connection with personal data which ________ ("you" or "the Controller") holds and processes in relation to me. By this letter I make a formal request for erasure of my personal data pursuant to Article 17 of the retained EU law version of the General Data Protection Regulation ("UK GDPR") and the Data Protection Act 2018.
