TREASURER'S AFFIDAVIT
(Increase of Authorized Capital Stock)

I, ________, of legal age, ________, with residence at ________, after having been duly sworn in accordance with law, do hereby depose and state THAT:

§ 1. I am the duly elected and qualified Treasurer of ________ (the "Corporation"), a corporation duly organized and existing under and by virtue of the laws of the Republic of the Philippines, with principal office address at:

§ 2. As Treasurer of the Corporation, and pursuant to Section 14 in relation to Section 37 of Republic Act No. 11232, otherwise known as the "Revised Corporation Code of the Philippines," I am duly authorized to receive, in trust and for and on behalf of the Corporation, all subscription and payment of the subscribers to the increase of its authorized capital stock.

§ 3. The Board of Directors and the stockholders of the Corporation, by a vote of at least two-thirds (2/3) of the outstanding capital stock, have duly approved the increase of the authorized capital stock of the Corporation from ________ (₱________) to ________ (₱________), or an increase of ________ (₱________), at a meeting duly called and held for the purpose on ________.

§ 4.
