AFFIDAVIT OF CHILD SUPPORT

2. I hereby promise and undertake to support by providing the needs of the child's psychological, emotional, moral and social well-being, as well as love, support, and giving the child the time and comfort I can provide.

4. Medical Attendance, and
5. Education and transportation;
