AFFIDAVIT OF SUPPORT AND UNDERTAKING
I, ________, Filipino, of legal age, ________, with residence and postal address at ________, after having been duly sworn to in accordance with law, do hereby depose and state THAT:
§ 1. Parentage. I am the natural/legitimate father of the following minor child (hereinafter, the “Child”):
________, Male, born on ________ in ________, as evidenced by Certificate of Live Birth bearing Registry No. ________;
§ 2. Legal Obligation to Support. Pursuant to Articles 195, 196 and 201 of the Family Code of the Philippines (Executive Order No. 209, as amended), and in conjunction with Articles 290 to 297 of the Civil Code of the Philippines (Republic Act No. 386), I acknowledge my legal and moral obligation to provide support to the Child, and I hereby freely and voluntarily promise and undertake to provide for the Child’s sustenance, dwelling, clothing, medical attendance, education and transportation, as well as for the Child’s psychological, emotional, moral and social well-being, in keeping with my financial capacity and the needs of the Child.
§ 3. Scope of Support. The support which I undertake to provide shall comprise everything indispensable for the following, in conformity with Article 194 of the Family Code:
3.1 Sustenance;
3.2 Shelter and dwelling;
3.3 Clothing;
3.4 Medical and dental attendance;
3.5 Education, schooling or training for some profession, trade or vocation, including transportation in keeping therewith.
§ 5. Amount, Period and Manner of Payment. In order to provide the support specified above, I promise and undertake to remit to the mother and custodial parent of the Child, ________, the sum of Philippine Pesos ________ (₱________) per month. The said obligation shall commence on ________ and shall be paid on or before the 1st (first) day of each calendar month, and shall continue until the Child reaches the age of majority or otherwise ceases to be entitled to support under law.

§ 8. Voluntariness. I am executing this Affidavit of Support and Undertaking freely and voluntarily, without any force, intimidation, fraud or undue influence from any person, and with full understanding of the nature and consequences of the obligations herein assumed.
§ 9. Purpose. I am executing this Affidavit to attest to the truth of the foregoing statements and to formalize my voluntary commitment and undertaking to provide support for the above-named Child, and for whatever legal purpose this instrument may serve.
