1. That we are a man and a woman, both of legal age, and that we are not suffering from any legal impediment to marry each other, neither of us being bound by any prior subsisting marriage nor falling within the prohibited degrees of relationship under Articles 37 and 38 of the Family Code of the Philippines;

2. That we have been living together as husband and wife, openly and continuously, without the benefit of a valid marriage, for a period of ________, that is, from ________ up to the present;
