TO: The heirs at law, devisees, legatees, and all other persons claiming or who may claim an interest in the Estate of the above-named decedent.

§ 1. Notice of Death and Interest. This is formal notice that ________ (the “Decedent”), who resided at the time of death at ________, died on ________. You have or may have an interest in the Decedent’s estate (the “Estate”) as an heir, devisee, or other interested person.

§ 2. Appointment of Personal Representative. Pursuant to Letters of Administration (or Letters Testamentary) issued by this Court on ________, the following individual has been duly appointed and qualified as the personal representative of the Estate:
