To Whom It May Concern:

§ 1. Purpose of Letter. I am writing to formally request a full refund in connection with the cancellation of my flight, identified as flight number ________, scheduled to depart from ________ on ________ at ________, with intended arrival at ________. The above-referenced flight was canceled by the carrier, and as a result I was unable to complete my itinerary as ticketed.

§ 2. Booking Details. I purchased this ticket on ________ through ________, using the following method of payment: ________. The total amount paid for the ticket was $________ (________).

§ 3. Statutory and Regulatory Basis for Refund. Under the regulations of the United States Department of Transportation ("DOT") and the rules implementing 49 U.S.C. § 41712 prohibiting unfair and deceptive practices, a consumer is entitled to a refund of the fare and ancillary fees when the carrier cancels a flight, or makes a significant change to a flight, and the consumer does not accept the alternative offered. The refund must be issued to the original form of payment, in the original currency, promptly and without the imposition of additional fees. Accordingly, I do not accept any voucher, travel credit, or rebooking in lieu of a cash refund, and I hereby demand a full refund of the fare and all associated charges.

§ 4. Demand for Refund of Fare. I request a full refund of the ticket price in the amount of $________ (________), together with all taxes, surcharges, and ancillary fees paid in connection with this reservation.
