§ 1. Purpose of Letter. I am writing to formally request a full refund in connection with the cancellation of my flight, identified as flight number ________, scheduled to depart from ________ on ________ at ________, with intended arrival at ________. The above-referenced flight was canceled by the carrier, and as a result I was unable to complete my itinerary as ticketed.

§ 3. Statutory and Regulatory Basis for Refund. Under the regulations of the United States Department of Transportation ("DOT") and the rules implementing 49 U.S.C. § 41712 prohibiting unfair and deceptive practices, a consumer is entitled to a refund of the fare and ancillary fees when the carrier cancels a flight, or makes a significant change to a flight, and the consumer does not accept the alternative offered. The refund must be issued to the original form of payment, in the original currency, promptly and without the imposition of additional fees. Accordingly, I do not accept any voucher, travel credit, or rebooking in lieu of a cash refund, and I hereby demand a full refund of the fare and all associated charges.
