1. I am writing further to the disciplinary hearing which you attended on ________, held in accordance with the Company's disciplinary procedure and having regard to the Acas Code of Practice on Disciplinary and Grievance Procedures. You were advised in advance, in writing, of the nature of the matters to be considered and of your statutory right under section 10 of the Employment Relations Act 1999 to be accompanied at the hearing by a trade union representative or a fellow worker of your choice.
