(1). We are parties to the above-named Contract. This notice is sent by ________ (party to the Contract).

Particulars of Breach

(2). This notice is sent in order to provide ________ with a formal Notice of Breach. In particular, there has been a failure to pay the sum of £________ (________) which, at the time of sending this notice, is late by ________ days.

(3). Take notice that this is considered to constitute a failure to comply with a fundamental element of the Contract.

(4). Please find enclosed a Letter of Demand. This sets out the full details of the debt, which should be read in conjunction with this notice.
