Re: Notice of Retirement and Resignation


Dear ________,

I, ________, holder of the position of ________ at ________ (the "Company"), and bearing Employee Identification No. ________, respectfully give written notice of my voluntary retirement from, and consequent separation from employment with, the Company.

I commenced employment with the Company on ________, and as of the effective date of this notice I will have rendered ________ years of continuous service. As I have attained the age of ________, I respectfully avail myself of retirement in accordance with Article 302 (formerly Article 287) of the Labor Code of the Philippines, as amended by Republic Act No. 7641 (the Retirement Pay Law), and any applicable Collective Bargaining Agreement, retirement plan, or Company policy.

In observance of the notice period of ________ required under my employment contract and/or Company policy, and consistent with Article 300 (formerly Article 285) of the Labor Code of the Philippines, please be advised that my retirement and resignation shall take effect on ________ (the "Effective Date").

I likewise request the issuance of my Certificate of Employment in accordance with Article 34(2) of the Labor Code and Labor Advisory No. 06, Series of 2020, as well as the appropriate BIR Form 2316 and any other documents necessary for the proper settlement of my accounts and the processing of my Social Security System (SSS), Pag-IBIG, and PhilHealth records.
