I, the undersigned, in my capacity as Lessor (the “Lessor”) under that certain Contract of Lease dated ________ (the “Lease”) entered into with you as Lessee (the “Lessee”), respectfully serve upon you this formal Notice of Lease Violation and Demand pursuant to the provisions of the Lease, the relevant provisions of the Civil Code of the Philippines (Republic Act No. 386), particularly Articles 1654, 1657, 1659, and 1673 thereof, and Republic Act No. 9653 (the Rent Control Act of 2009, as extended), where applicable.

§ 1. The Leased Premises. The Lease covers the real property more particularly described as follows (the “Premises”):
