Dear ________,
§ 1. Purpose of this Notice. I, ________, of legal age and presently employed with ________ as ________ (Employee Number: ________), respectfully write to formally notify you of my intention to avail myself of the additional maternity leave of Thirty (30) Days, without pay, in addition to the One Hundred Five (105) Days of paid maternity leave I am currently enjoying in connection with my live childbirth on ________.
§ 3. Period of Extension. Accordingly, I hereby give notice that I intend to extend my maternity leave by Thirty (30) Days, without pay, to commence immediately upon the expiration of my current maternity leave.
§ 4. Timeliness of Notice. My current maternity leave is scheduled to end on ________. The additional leave shall therefore run from ________ until ________, and I intend to resume the performance of my duties on ________. This notice is being served on you not later than Forty-Five (45) Days before the end of my maternity leave and is thus well within the period prescribed by law.
§ 5. Offsetting Against Accrued Leave Credits. Mindful that the additional Thirty (30) Days is granted without pay under Republic Act No. 11210, and in accordance with the company policy and the applicable provisions of our Collective Bargaining Agreement and/or Employee Handbook, I respectfully request that the said additional maternity leave be offset against my earned and unused leave credits, if any, in the following manner:

Should my available leave credits be insufficient to cover the entire period of extension, I respectfully confirm my understanding that the unoffset portion shall be considered as leave without pay, consistent with Section 3(a)(3) of Republic Act No. 11210.
§ 7. Supporting Documents. Attached herewith, for your records and reference, are the following: (i) a copy of my Notice of Allocation/Initial Maternity Leave Application previously filed; (ii) my medical certificate and proof of childbirth; and such other documents as you may reasonably require, namely ________.
