Re: Demand for Necessary Repairs and Maintenance of Leased Premises

I, ________, of legal age, with residence address at ________, am the lessee of the property described below under that certain Contract of Lease dated ________ (the “Lease”) executed between us. I respectfully write to formally notify you of, and to demand, the necessary repairs on the following leased premises:
