I write in my capacity as a regular employee of ________ (the he position of ________ under the ________, to respectfully request the grant of a temporary flexible working arrangement in accordance with the policies of the Company and applicable Philippine labor law and issuances, including the Labor Code of the Philippines (Presidential Decree No. 442, as amended and renumbered) and the relevant Advisories and Labor Advisories of the Department of Labor and Employment (DOLE), in particular DOLE Department Advisory No. 02, Series of 2009 (Guidelines on the Adoption of Flexible Work Arrangements) and DOLE Labor Advisory No. 04, Series of 2010.

I.  Background and Reason for the Request. I sincerely value my employment with the Company and remain fully committed to its continued success. I am constrained to make this request for the following bona fide reason/s:
