Dear ________,


I, ________, of legal age, with residence and postal address at ________, write through this letter to formally and unequivocally demand that you immediately CEASE AND DESIST from the unlawful, unwelcome, and injurious acts hereinafter described, which you have committed and continue to commit towards and against me.

I. The Offensive Acts. It has come to my attention that, beginning on or about ________ and continuing to the present, you have engaged in the following acts:
