RE: APPLICATION FOR CONSENT TO ASSIGN LEASEHOLD INTEREST IN THE PROPERTY SITUATE AT ________

§1. Introduction. I, the undersigned Tenant/Lessee, hereby formally apply for your written consent, as Landlord/Lessor, to assign the unexpired residue of my term in the leasehold interest held by me in the property situate at and known as ________ (the “Property”), to ________ of ________ (the “Proposed Assignee”).

§2. Particulars of the Lease.

  1. The Deed of Lease/Tenancy Agreement in respect of the Property is dated ________ (the “Lease”), and was duly executed between the parties.
