上記の賃貸人と賃借人は、________付け________（これに加えられた追加・修正・変更・更新等がある場合はそれを含む。以下「原契約」という。）を下記のとおり更新することに合意し、これを証するため本覚書を作成する。
第1条 賃貸借の更新
1.1 賃貸人と賃借人は、________をもって期間満了となる原契約に基づく賃貸借を、 ________まで更新する。
1.2 本条に基づき賃貸借を更新するにあたり、賃借人は、賃貸人に対して、更新料として金________(________)円を支払う義務を負い、これを本覚書締結時に支払うものとする。
第2条 更新後の賃料
更新後の賃料は、更新前の賃料と同額とする。
第3条 原契約条項の変更
原契約の条項を次のとおり変更する。
