§ 1. Purpose of Notice. This letter constitutes formal written notice (the “Notice”) that the undersigned tenant (“Tenant”) intends to vacate and surrender possession of the rental premises identified below, and that Tenant requests the landlord’s consent to assign or sublet the remainder of the lease term to a proposed replacement tenant, all in accordance with the terms of the lease and applicable state and local law.

§ 2. The Lease. Tenant occupies the Premises pursuant to a written residential lease agreement dated ________ (the “Lease”), with a term commencing on ________ and scheduled to terminate on ________. The current monthly rent under the Lease is ________, and Tenant previously deposited a security deposit in the amount of ________.

§ 3. Notice of Intent to Vacate. Tenant hereby gives notice that Tenant is unable to remain in possession of the Premises for the full balance of the Lease term, and intends to vacate and surrender the Premises on ________ (the “Vacate Date”). Please advise in writing, no later than ________, whether the Proposed Replacement Tenant is acceptable.

§ 6. Security Deposit. Tenant requests that the landlord conduct any move-out inspection and provide an itemized written statement of any deductions, together with the return of the remaining security deposit, within the time period required by applicable state law.
