RE: NOTICE OF RESIGNATION

§ 1. I am writing to formally tender my resignation from my position as ________ with ________ (the "Company"), which I have held since ________. My employee/payroll reference number is ________.

§ 2. In accordance with the notice provisions of my contract of employment dated ________, and subject to the minimum statutory notice requirements set out in section 86 of the Employment Rights Act 1996, I hereby give the Company notice of my resignation. The notice period applicable to me is: ________.

§ 3. On the basis of the foregoing notice period, I calculate that my last day of employment with the Company will be ________ (the "Termination Date"). I confirm that I will continue to work and to discharge my duties diligently throughout my notice period, save where the Company elects to place me on garden leave or to make a payment in lieu of notice where the contract so permits.
