§ 1. Reference to the Lease. Reference is made to that certain Contract of Lease executed by and between the Lessor and the Lessee on ________ (the "Lease"), covering the real property described as follows: ________ (the "Leased Premises"). The term of the Lease commenced on ________ and was to expire on ________.

§ 2. Notice of Termination. Pursuant to Article 1673 and the relevant provisions of the Civil Code of the Philippines, and in accordance with the stipulations of the Lease, the Lessee hereby formally notifies the Lessor of his/her/its intention to terminate the Lease and to vacate and surrender the Leased Premises prior to the original expiry date. This written notice is given in observance of the notice period of ________ required under the Lease.

§ 3. Date of Surrender. The Lessee shall vacate, peaceably surrender, and deliver possession of the Leased Premises to the Lessor on ________ (the "Effective Date of Surrender"). Upon such surrender, the Lessee shall be released from all obligations under the Lease that accrue after the Effective Date of Surrender, save for any liabilities that may have accrued on or before said date.

§ 6. Return of Security Deposit. In accordance with the Lease, the Lessee respectfully requests that the security deposit in the amount of ________ (₱________) be refunded to the Lessee within ________ from the Effective Date of Surrender, less only such lawful deductions, if any, for unpaid obligations or for documented damage to the Leased Premises beyond ordinary wear and tear. Any deduction shall be supported by an itemized statement of account furnished to the Lessee.

§ 7. Inspection and Turnover. The parties shall conduct a joint inspection and turnover of the Leased Premises on or before the Effective Date of Surrender, at which time a turnover acknowledgment shall be executed evidencing the Lessor's acceptance of possession.

§ 8. Reservation of Rights. This notice is given without prejudice to, and with full reservation of, all rights, claims, and remedies available to the Lessee under the Lease and applicable law.
