I, ________, of legal age, with address at ________, respectfully write to formally request your permission, as the owner and/or holder of the rights to the intellectual property described below, to use the same in accordance with the Intellectual Property Code of the Philippines (Republic Act No. 8293, as amended).

§ 5. License Terms and Fees. Should you be willing to grant permission, I would welcome the opportunity to enter into a written licensing agreement setting out the terms of use. Kindly advise me in your reply of any license fee, royalty, or condition you may wish to impose, in the amount of ________ or such other amount as we may mutually agree.

§ 6. Written Response. I would be most grateful if you could confirm in writing whether you grant me permission to use the Work, and on what terms. I understand that no permission shall be deemed granted unless and until it is given by you expressly and in writing, and that the absence of a reply shall not constitute consent.
