I, ________, of legal age and presently residing at ________, in my capacity as Lessee under that certain Contract of Lease dated ________ (the "Lease Agreement") executed by and between us, respectfully write to formally notify you, in your capacity as Lessor, of my intention to renew the lease over the following described property (the "Leased Premises"):

§ 1. Purpose of the Lease. The Leased Premises are leased, and are intended to continue to be used, primarily for residential purposes.


§ 2. Expiration of Current Term. The current term of the Lease Agreement is set to expire on ________. Pursuant to the terms of the Lease Agreement, the Lessee is required to communicate any intention to renew the lease within the period of ________ prior to the expiration of the existing term. This Notice is therefore submitted within the period so prescribed.


§ 3. Intention to Renew. I hereby manifest my intention to renew the lease over the Leased Premises for a further period of ________, commencing on ________, under the same terms and conditions stipulated in the existing Lease Agreement, including a monthly rental of ________. I am, however, equally willing to negotiate and agree upon new or revised terms and conditions for the renewal, subject to mutual consent.


§ 4. Request for Written Response. I respectfully request that you signify in writing, on or before ________, whether the lease may be renewed under the existing terms and conditions. Should you agree, I propose that we execute a written contract of renewal of lease, duly notarized, to formalize the same. Alternatively, should you wish to revise any of the terms, I propose that we set a mutually convenient date for negotiation thereof.
