Dear ________,

I write to you in my capacity as the lessor (and/or duly authorized representative of the lessor) of the real property more particularly described as follows (the "Leased Premises"):

________

§1. The Lease. The Leased Premises are the subject of that certain Contract of Lease executed by and between us on ________ (the "Lease"), pursuant to Articles 1642 to 1688 of the Civil Code of the Philippines (Republic Act No. 386). Under the Lease, you bound yourself to pay a monthly rental of ________ (\u20b1________), payable on or before the ________ of each calendar month.

§2. Default. Records show that your last rental payment was made on ________. As of the date of this letter, you have failed and refused to pay ________ monthly rentals, in the aggregate amount of ________ (\u20b1________). By reason of such default, you have likewise incurred penalties and/or interest in the amount of ________ (\u20b1________), as stipulated in the Lease. Accordingly, your total outstanding obligation as of this date amounts to ________ (\u20b1________).

§3. Final Demand. In view of the foregoing, and pursuant to Article 1673 of the Civil Code and Section 2 of Rule 70 of the 1997 Rules of Civil Procedure, as amended, I hereby make FINAL and FORMAL DEMAND upon you to: (a) PAY the total sum of ________ (\u20b1________); AND (b) VACATE and PEACEFULLY SURRENDER the Leased Premises, together with all keys and accessories thereto, both within FIFTEEN (15) DAYS from your receipt of this letter.

§6. Inquiries. Should you have any questions or wish to discuss an amicable settlement of this matter, you may reach me at ________.

I trust that you will give this matter your immediate and serious attention to avoid any further legal action. I look forward to your prompt compliance with this demand.

Very truly yours,
