Dear Sir/Madam,
I, ________, of legal age, with residence and postal address at ________, holder of ________ bearing No. ________, hereby respectfully submit this formal written demand for compensation on account of the damage sustained by my checked-in baggage while in the custody and care of your airline.
§ 1. Particulars of the Carriage. The relevant details of the flight in question are as follows:

§ 2. The Incident. Upon arrival at the destination airport, I retrieved my checked-in baggage from the conveyor belt and found the same to be damaged while in the exclusive custody and control of your airline during the carriage. The nature and extent of the damage, together with the value thereof, are described as follows:

§ 3. Notice and Report. Immediately upon discovery of the damage, and well within the period prescribed under Article 31 of the Convention for the Unification of Certain Rules for International Carriage by Air (the "Montreal Convention"), I lodged a Property Irregularity Report (PIR) / baggage claim form bearing No. ________ with the duly authorized representatives of ________, a copy of which is attached hereto as Annex "B". Notwithstanding said report, no satisfactory remedy or compensation has been offered to date.
