AFFIDAVIT OF SUPPORT WITH UNDERTAKING

§ 1. Personal Circumstances and Employment. I am gainfully employed in the position of ________ with ________, a duly organized entity with business address at ________;

§ 2. Income. I am currently earning a monthly income of ________, and I have sufficient financial capacity and resources to undertake the obligations herein set forth;

§ 3. Invitation. I am inviting my ________, ________ (hereinafter referred to as the "Invitee"), to visit me in ________ for a period of ________;

§ 4. Identity of the Invitee. The Invitee was born on ________ at ________, is a citizen of ________, and is currently residing at ________;

§ 5. Travel Document. The Invitee holds Passport No. ________, issued on ________ at ________, valid until ________;

§ 6. Accommodation. During the entire duration of the visit, the Invitee shall stay with me at my residence located at ________;

§ 7. Undertaking as to Expenses. I voluntarily and freely bind myself to provide for, and to shoulder, all expenses of the Invitee, including but not limited to round-trip airfare, board and lodging, food, medical and hospitalization expenses, incidental costs, and all other reasonable expenses incident to the Invitee's travel to and sojourn in ________;

§ 8. Guarantee Against Public Charge. I hereby guarantee and undertake that the Invitee shall not become a public charge or burden upon the government or any institution of ________ at any time during the Invitee's stay;

§ 9. Undertaking as to Repatriation and Compliance. I further undertake to ensure that the Invitee shall faithfully comply with the immigration laws and regulations of ________, shall not engage in any unauthorized employment or activity, and shall depart said country on or before the expiration of the authorized period of stay; and I bind myself to assume full responsibility for, and to defray the costs of, the repatriation of the Invitee to the Philippines or to the Invitee's country of residence should the need arise;
