JOINT AFFIDAVIT OF LEGITIMATION

WE, ________, Filipino, of legal age, and ________, Filipino, of legal age, spouses, presently residing at ________ and ________, respectively, after having been duly sworn to in accordance with law, hereby depose and state:

1. That we are the biological father and mother of the following child, namely:

________, Male, born on ________ at ________, and whose birth was registered with the Office of the Local Civil Registrar of ________ under Registry No. ________ (hereinafter referred to as our "Child");

2. That at the time of the conception and birth of our Child, both of us were not married to each other but were not disqualified by any legal impediment to marry each other, neither party being then under any subsisting marriage nor otherwise barred by Articles 37 and 38 of the Family Code of the Philippines from contracting marriage;

3. That our Child was begotten and born out of wedlock;

4. That subsequent to the birth of our Child, we were lawfully and validly married to each other on ________ at ________, as evidenced by our Certificate of Marriage registered with the Office of the Local Civil Registrar of ________ under Registry No. ________;

6. That we hereby acknowledge and recognize the above-named Child as our own, legitimated by the said subsequent marriage;
