§ 3. The Business was issued a Business Permit/Mayor's Permit on ________ by the Office of the Mayor of ________, under Permit No. ________, pursuant to the Local Government Code of 1991 (Republic Act No. 7160);

§ 4. The Business was registered with the Bureau of Internal Revenue (BIR) on ________, in accordance with Section 236 of the National Internal Revenue Code of 1997, as amended, under Certificate of Registration OCN No. ________;
