Dear Sir/Madam,

RE: FORMAL REQUEST FOR MAINTENANCE AND REPAIRS TO THE DEMISED PREMISES SITUATE AT ________

§1. Introduction. I write in my capacity as the tenant in lawful occupation of the premises situate at and known as ________ ("the Premises"), to make a formal demand for the maintenance and repairs hereinafter particularised to be carried out on the Premises.

§2. Particulars of the Tenancy. The Premises were let to me pursuant to a tenancy agreement dated ________ at a rent of ________ per ________. I have been in lawful and continuous possession of the Premises since ________, and my rent is paid up to ________.

§4. Particulars of Repairs Required. Specifically, the following repairs are required to be carried out:

§6. Right of Entry and Notice. I confirm my willingness to afford you or your duly authorised contractors reasonable access to the Premises for the purpose of effecting the repairs, provided that you furnish me with not less than ________ prior written notice of the intended date and time of entry, in accordance with the tenancy agreement and the applicable provisions of the Tenancy/Recovery of Premises Law of ________.

§7. Demand and Time for Compliance. I kindly request that the aforesaid repairs be carried out at your earliest convenience and, in any event, within fourteen (14) days from the date of this letter. Time shall be of the essence in this regard.

§8. Reservation of Rights. TAKE NOTICE that should you fail, refuse or neglect to respond to this letter or to take appropriate action within the said period of fourteen (14) days, I shall be at liberty, without further notice or reference to you, to pursue all such remedies as are available to me under the tenancy agreement and the laws of ________, including (where permitted) effecting the repairs and setting off the reasonable costs thereof against accruing rent, and/or seeking such relief as a court of competent jurisdiction may grant. All my rights in this regard are expressly reserved.

§9. I thank you for your anticipated cooperation and look forward to your prompt action in this matter.
