I, ________, son/daughter/wife of ________, aged ________ years, holding Designated Partner Identification Number (DPIN): ________, being the Designated Partner of ________ (hereinafter referred to as the "LLP"), a limited liability partnership incorporated on ________ bearing Limited Liability Partnership Identification Number (LLPIN): ________ under the provisions of the Limited Liability Partnership Act, 2008, and having its registered office at ________, do hereby solemnly affirm and sincerely declare as under:

§ 1. That I am the Designated Partner of the LLP since ________ and am duly authorised by a resolution of the partners of the LLP dated ________ to swear and affirm this Affidavit on behalf of the LLP, and I am competent and acquainted with the facts and affairs of the LLP stated herein.

§ 2. That my permanent residential address is ________.

§ 3. That my present residential address is ________, and my Permanent Account Number (PAN) is ________.

§ 4. That the LLP has ceased to carry on its business or commercial operations with effect from ________, and is not carrying on any business or operations as on the date of this Affidavit.

§ 5. That the LLP has filed its income tax returns under the Income-tax Act, 1961 up to and including the financial year ________, and has discharged all statutory dues and filings to the extent applicable as on the date hereof.

§ 6. That as on the date of this Affidavit, the LLP has an outstanding liability/debt amounting to Rs. ________ (Rupees ________), and the said liability/debt shall be fully discharged and paid off within a period of ________ days from the date of swearing this Affidavit.
