RE: PROTECTED DISCLOSURE OF WRONGDOING IN THE WORKPLACE (WHISTLEBLOWING)
§1. Purpose of this letter
1.1. I am writing to raise a whistleblowing concern and to make a qualifying disclosure of information which I reasonably believe tends to show wrongdoing of the kind described below.
1.2. I make this disclosure pursuant to Part IVA (sections 43A to 43L) of the Employment Rights Act 1996, as inserted by the Public Interest Disclosure Act 1998, and in accordance with ________'s whistleblowing policy.
1.3. I am raising this concern in my capacity as ________ and am a worker within the meaning of section 43K of the Employment Rights Act 1996.

§3. Qualifying and protected disclosure
3.1. I make this disclosure as a qualifying disclosure under section 43B of the Employment Rights Act 1996. I reasonably believe that the disclosure is made in the public interest.
3.2. I reasonably believe that the information disclosed tends to show one or more of the following matters set out in section 43B(1) of the Employment Rights Act 1996: ________ (which may include that a criminal offence has been, is being or is likely to be committed; that a person has failed, is failing or is likely to fail to comply with a legal obligation; that a miscarriage of justice has occurred, is occurring or is likely to occur; that the health or safety of any individual has been, is being or is likely to be endangered; that the environment has been, is being or is likely to be damaged; or that information tending to show any of these matters has been, is being or is likely to be deliberately concealed).
3.3. I make this disclosure to you as my employer in accordance with section 43C of the Employment Rights Act 1996 and the relevant internal procedure.
3.4. I am also concerned that attempts may have been or are being made to conceal the wrongdoing described above.
§4. Request for action
4.1. I request that you investigate the matters set out above promptly and as soon as reasonably practicable, in accordance with the whistleblowing policy.
4.2. I would be grateful if you would acknowledge receipt of this letter and keep me informed of the steps taken, the progress of any investigation and its outcome, so far as it is appropriate and lawful to do so.
§5. Protection from detriment
5.1. I trust that I will not be subjected to any detriment, victimisation or dismissal as a result of having made this disclosure.
§6. Confidentiality and data protection
6.1. Please treat this letter and its contents as confidential. I ask that my identity and the details of this disclosure are not disclosed beyond those who need to know them for the purpose of investigating and addressing the matters raised.
6.2. Any personal data contained in this letter should be processed in accordance with the UK General Data Protection Regulation and the Data Protection Act 2018.
