RE: FORMAL GRIEVANCE – COMPLAINT OF SEXUAL HARASSMENT
§ 1. Purpose of this letter. I am writing to raise a formal grievance, in accordance with the organisation’s grievance and anti-harassment procedures and the ACAS Code of Practice on Disciplinary and Grievance Procedures, in respect of sexual harassment to which I have been subjected by ________ (whose role within the organisation is ________) in the course of my employment with ________.
§ 2. My employment details. I commenced employment with the organisation on ________ and I am currently employed in the position of ________, reporting to ________.
§ 3. Nature of the conduct complained of. The conduct of which I complain amounts to unwanted conduct of a sexual nature, and/or conduct related to sex, which has had the purpose or effect of violating my dignity and/or of creating an intimidating, hostile, degrading, humiliating or offensive environment for me. The harassment first occurred on or about ________ and has continued during the performance of my duties.
