RE: Notice of Cancellation of Contract under the Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013

1. I am writing to give you formal notice that I hereby cancel my contract with ________ ("you"). I exercise this right of cancellation in accordance with regulations 27 to 38 of the Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013 (the "Regulations").

2. The particulars of the contract I wish to cancel are as follows:
