Re: Notice of Retirement and Resignation

I, ________, holder of the position of ________ at ________ (the "Company"), and bearing Employee Identification No. ________, respectfully give written notice of my voluntary retirement from, and consequent separation from employment with, the Company.
I commenced employment with the Company on ________, and as of the effective date of this notice I will have rendered ________ years of continuous service.
