We write in respect of the above-named event and, in accordance with the terms of the agreement and/or ticket conditions governing the same, hereby give you formal notice as follows:

1. Notice of Cancellation

2. Reason for Cancellation
   The cancellation has become necessary for the following reason(s):

Any approved refund shall be processed and remitted within ________ of receipt of your valid request.
