Dear Sir / Madam,

§1. Purpose of this Notice. I, the undersigned, hereby serve this written notice upon ________ (the “Service Provider”) to formally communicate my decision to cancel and terminate my Membership with the Service Provider, in exercise of my rights under the agreement entered into between us dated ________ (the “Agreement”) and in accordance with the provisions of the Indian Contract Act, 1872 and all other applicable laws.

§2. Particulars of the Membership. The Membership sought to be cancelled is identified by Membership / Account No. ________, registered in the name of ________, and commenced on ________.

§3. Reason for Cancellation. I have decided to cancel my Membership for the following reason(s):

§4. Effective Date of Cancellation. I request that this cancellation take effect on and from ________ (the “Effective Date”). With effect from the Effective Date, I shall stand discharged from all obligations under the Agreement, save for any liability that has accrued prior to the Effective Date, and the Service Provider shall cease to debit, charge, or collect any further amount, fee, subscription, or instalment from me or from any payment instrument, mandate, or auto-debit arrangement standing in my name.

§5. Cancellation Fee. Without admission of any liability, and strictly in terms of the Agreement, I enclose / undertake to remit the applicable cancellation fee of Rs. ________, if and to the extent the same is lawfully and validly chargeable under the Agreement.

§6. Refund of Balance. Please refund the unutilised / remaining balance on my Membership amounting to Rs. ________ by way of Cheque made out to ________, or by direct credit to my bank account bearing Account No. ________, ________, within ________ days from the date of receipt of this notice.

§7. Written Confirmation. Kindly send me written confirmation of the cancellation of my Membership, together with a final statement of account, within thirty (30) days of receipt of this letter, at the postal or email address mentioned herein.
