RE: Request for Repairs and Maintenance — Rental Unit located at ________

§ 1. Tenancy Particulars. I am the tenant of the residential premises located at ________ (the "Rental Unit"), occupied pursuant to a residential tenancy agreement dated ________. This letter constitutes my formal written request, made under the residential tenancy legislation of the Province of ________, that you carry out the repairs and maintenance described below.

§ 3. Date Defect First Noticed. The condition described above was first observed by me on or about ________. ________
§ 4. Landlord's Statutory Obligation. Under the applicable residential tenancy legislation of the Province of ________, you are required to provide and maintain the Rental Unit in a good state of repair and fit for habitation, and to comply with all applicable health, safety, housing and maintenance standards, regardless of whether you were aware of any defect before the tenancy commenced. The repair(s) requested above fall within this obligation.
§ 5. Request to Complete the Repair. I respectfully request that you arrange for the above repair(s) to be completed within a reasonable time, and in any event no later than ________, or sooner if the nature of the defect requires urgent attention.
§ 6. Notice of Entry. Should you or a person acting on your behalf (including any contractor or tradesperson) need to enter the Rental Unit to inspect or carry out the repair(s), I require that, except in the case of an emergency, you provide me with proper written notice of entry in accordance with the residential tenancy legislation of the Province of ________. I understand that such notice must specify the reason for entry and the date and time of entry, that entry must occur between reasonable hours, and that the minimum notice period required by law is ________. I would appreciate written confirmation of the proposed date and time of entry so that I may make appropriate arrangements to provide access.
§ 7. Reservation of Rights. Nothing in this letter waives or limits any of my rights or remedies as a tenant under the residential tenancy legislation of the Province of ________ or at common law. If the repair(s) are not completed within a reasonable time, I reserve the right to pursue all remedies available to me, which may include applying to the applicable residential tenancy tribunal or board for an order requiring the repair, an abatement of rent, compensation, and/or such other relief as may be appropriate.
