Dear Tenant:

§ 1. Purpose of this Notice. This letter is delivered to you for the purpose of giving you formal written notice that you are in breach of your obligations under the residential tenancy agreement dated ________ (the "Lease") in respect of the Rental Unit, and to require that you remedy that breach within the time set out below.

§ 2. Description of the Breach. The breach of the Lease consists of the following:
