RE: NOTICE OF ABSENCE AND REQUEST FOR PAID PERSONAL/CARER’S (SICK) LEAVE


Dear ________,

I am writing to provide formal written notice, in accordance with Division 7 of Part 2-2 of the Fair Work Act 2009 (Cth) and the terms of my contract of employment, that I am unable to attend work and wish to take paid personal/carer’s (sick) leave.

§ 1. Particulars of Absence

1.1 My absence will commence on ________.

1.2 I anticipate returning to work on ________, which represents a total of ________ working days. Should my circumstances change, I will provide you with further notice as soon as practicable.

1.3 I confirm that, where notice could not be given in advance of my absence, this notice is provided as soon as reasonably practicable in accordance with section 107 of the Fair Work Act 2009 (Cth).

§ 2. Reason for Leave

2.2 The nature of my circumstances is as follows: ________.

2.3 I confirm our discussion of ________, during which you indicated your verbal acknowledgement of my intended absence. This letter is provided as a formal written record of that discussion and of my request.

§ 3. Evidence in Support
