1.1 I refer to my employment with ________ (the Employer) in the position of ________, which commenced on ________.

1.2 I am writing to follow up on the discussion we had on ________ in relation to my request for a flexible working arrangement, namely to work from home.

2. Statutory basis of my request

2.1 My request is made pursuant to section 65 of the Fair Work Act 2009 (Cth) (the Act), which entitles eligible employees to request a change in their working arrangements.

2.2 I confirm that I am eligible to make this request because I have completed at least 12 months of continuous service with the Employer, and I fall within one or more of the circumstances set out in section 65(1A) of the Act, being: ________.

2.3 The change I requested is to perform my duties from my home address located at ________, with effect from ________, on the following basis: ________.

3. Record of our discussion
