I refer to the tenancy agreement between us dated ________ in respect of the premises situated at ________ (the Premises), in the State or Territory of ________.
This letter is given as formal written notice that a defect requiring repair has been identified at the Premises. The defect was first observed by me on or about ________ and may be described as follows:

I hereby request that you carry out the following repairs to the Premises:

1. Nature of the defect. This defect was not apparent at the commencement of the tenancy or during the initial inspection of the Premises and has only since become evident. The defect was not caused, and has not been contributed to, by me, any member of my household, or any person at the Premises with my permission. The defect is in the nature of a latent or hidden defect and is now affecting the proper functioning and amenity of the Premises.
3. Timeframe for repairs. Where the repair is an urgent repair (including any repair posing a risk to health or safety, or causing significant inconvenience or potential further damage to the Premises), I request that it be attended to immediately and in any event within the period prescribed by the applicable legislation. Where the repair is a non-urgent repair, I request that it be completed within a reasonable time, being no later than ________, or such other period as is reasonable having regard to the nature of the defect (commonly within 14 to 28 days).
5. Access. I confirm that I will facilitate reasonable access to the Premises for the purpose of any inspection or the carrying out of repairs, upon reasonable prior notice being given to me in accordance with the applicable legislation.
