To whom it may concern,
I, ________, am the borrower / debtor under the credit contract identified by the account reference number set out above (the Credit Contract). I am writing to notify you that I am presently unable, or expect to be unable, to meet my repayment obligations under the Credit Contract by reason of financial hardship, and to formally request a variation of the terms of that contract.
I make this request as a notice of hardship pursuant to section 72 of the National Credit Code (being Schedule 1 to the National Consumer Credit Protection Act 2009 (Cth)) and in accordance with your obligations under that Act and the Australian Securities and Investments Commission Act 2001 (Cth).
