To whom it may concern,
§1 – Purpose of this Notice
1.1 This letter constitutes formal written notice of my election to cancel the subscription agreement (the “Subscription”) entered into between myself, ________ (the “Customer”), and ________ (the “Supplier”).
1.2 My relevant account details are as follows:

§2 – Cancellation
2.1 In accordance with the terms of the Subscription and my rights at law, I hereby cancel the Subscription with effect from ________ (the “Cancellation Date”).
2.2 I direct that, from the Cancellation Date, the Supplier:
(a) cease the provision of all services and products supplied under the Subscription;
(b) immediately cancel any recurring payment, direct debit or continuing payment authority associated with the Subscription, including any authority given to my financial institution or card issuer; and
(c) take no further payment from my account, credit card or any other payment method.
2.3 I withdraw, with effect from the Cancellation Date, any consent, authority or instruction previously given to the Supplier to debit or charge any of my accounts in connection with the Subscription.
§3 – Cancellation Fee
3.1 I have enclosed a cancellation fee in the amount of $________ (________).
§4 – Refund of Balance
4.1 Please refund the remaining credit balance on the Subscription in the amount of $________ (________), together with any pro-rata refund of amounts paid in advance for services not yet provided as at the Cancellation Date.

§5 – Consumer Guarantees and Statutory Rights
5.1 Nothing in this letter limits, excludes or modifies any guarantee, right or remedy available to me under the Australian Consumer Law or any other applicable law that cannot lawfully be excluded.
5.2 To the extent this cancellation relates to a service or product that did not meet a consumer guarantee, this letter is also given without prejudice to my rights to a remedy in respect of that failure.
