RE: Formal Demand for Payment of Past-Due Support — Case No. ________

§ 1. Purpose of Letter. This letter constitutes a formal demand for the payment of past-due support obligations owed by you (the “Obligor”) to ________ (the “Obligee”) pursuant to the support provisions described below.
