§ 1. Purpose of This Communication. This letter constitutes a formal, written complaint of workplace harassment that I have experienced in the course of my employment with ________ (the “Company”). I am submitting this complaint to place the Company on notice and to request that it discharge its legal obligation to promptly investigate and remediate the conduct described below. I respectfully request that this matter be treated as a formal complaint pursuant to the Company’s anti-harassment policy and applicable law, including Title VII of the Civil Rights Act of 1964, 42 U.S.C. § 2000e et seq., and the applicable anti-discrimination and anti-harassment statutes of the State of ________.
