Re: Notice of Complaint and Demand to Remedy Conditions at ________ (the “Premises”)
Dear ________:
§ 1. Tenancy. I am your tenant in lawful possession of the Premises located at ________, pursuant to that certain written lease agreement dated ________ (the “Lease”). I am current in my rental and other obligations under the Lease, except as may be excused by the conditions described herein.

In addition to the terms of the Lease, you are obligated under the implied warranty of habitability and the applicable landlord-tenant laws and housing/building codes of the State of ________ to maintain the Premises in a habitable, safe, and tenantable condition and to make all repairs necessary to that end. The condition described above constitutes a breach of those obligations.
§ 8. Demand and Cure Period. Accordingly, I hereby demand that you commence and complete all repairs and remedial measures necessary to correct the condition described above within ________ days from your receipt of this letter, or within such shorter period as may be required by law for emergency or health-and-safety conditions. Please take whatever steps are required to resolve this matter promptly so that I may continue to enjoy the Premises as provided under the Lease and applicable law.
